Why does my QR code not work when I print it?

I placed a QR code on a flyer I made in photoshop. Tested it while on the computer and it worked fine. After I printed it, it would not scan at all. I tried this 5 more times with different sized QRs, and still had no luck. What am I doing wrong? Does the paper quality play a roll?
